The focus for our work is to support the primary/community nursing workforce to be ‘fit for purpose’ to support the ‘care closer to home’ agenda. We have focused on all nurses in community care setting including general practice, nursing homes and community hospitals.

Areas of work include:

• Agreeing the ‘Role Functions’ for community/case managers

• Ensuring educational provision for community matrons is fit for purpose

• Development of a electronic competency assessment tool for use for community matrons

• Increasing the capacity of nursing workforce in general practice

• Practice Nurse meetings to address ‘key issues/projects’ as and when needed

• Working with providers of pre-registration nurse training to be more primary care focused

• Increasing primary care clinical placements

• An electronic resource called 'Nurses on the Move' toolkit to enable teams to support local service redesign and workforce development
