The LTC clinical pathway group is keen to ensure the whole LTC workforce is fit for purpose and practice across all health care providers. Areas of work include:

• Developing a E learning educational package giving key standardized messages for LTC management for all the workforce
• Development of a ‘Peer review programme for LTC teams’
• Care planning training for all the LTC workforce
• Supporting ‘Year of Care’ approach for all LTCs
• Supporting the workforce to use Telehealth care as part of increasing self management of patients with LTCs
• Resource pack to support local teams to develop competency based workforce for LTCs
• Ensuring education provision is underpinned with the core competences for LTCs and specialists/advanced competences depending on specialism
• QIPPP streams ‘Specialist community teams for LTCs’ and ‘Think Glucose’
